What Clinicians Should Know About the Use of Albumin

The advantages of albumin over less costly alternative fluids continue to be debated. Many scientific articles were devoted to the clinical analysis of the use of albumin in acute illness as well as its comparison with other fluid regimens. However, the lack of fundamental knowledge about the physical and chemical properties of commercial albumin generates many unpromising discussions about the effectiveness of the use of albumin among practitioners and medical scientists. The manuscript provides information about the different variants of commercial albumin, the mechanisms of their action, indications and contraindications to use. The presented information is based on fundamental knowledge of physical and chemical properties of commercial albumin. This manuscript is not only educational information, but also is guide to action for clinicians. Follow the link for full article https://medwinpublishers.com/HIJ/HIJ16000139.pdf

Expanding the Classification of Leukemia by the World Health Organization over Time

The classification has always been the basis for many treatments of the disease. The French-American-British has performed leukemia classification base on morphological finding, but the World Health Organization apply gene or chromosomal mutations in its classification. This approach has revolutionized the diagnosis, choose strategy of treatment and follow-up ever since. Although WHO has classified leukemia on the basis of FAB; but genetic mutations has made the wider and more specific outcome. Obviously, identifying these damages provide an extensive filed for researchers and physicians to effective assistance for patients. Therefore, the purpose of this review is comparing revisions of WHO classification in the years 2008 and 2016, because these categories are highlighted unknown cases that give researchers incentives to further study. Where do we Position Transfusion Medicine in the Family of Sciences?

Transfusion Medicine has come a long way and has for long been regarded a Cinderella in medicine and medical science. Since the discovery around 1900 by Karl Landsteiner of the ABO blood groups as principle elements for compatibility, the science has been dominated by the laboratory research for immunohematology, microbiology and virology, focused on the test tube and not so much the patient. Over the 20st century several development eras are to be recognized that contributed to the maturation of transfusion medicine as a bridging science. Twenty Nobel laureates contributed to this development. Follow the link for full article https://medwinpublishers.com/HIJ/HIJ16000137.pdf

Right Ventricular Function Assessment in Sickle Cell Anaemia Patients Using Echocardiography

Right ventricular dysfunction has been linked to poor prognosis in people with sickle cell anaemia (SCA). There is however a paucity of studies on right ventricular function in SCA. The objectives were to determine right ventricular function in subjects with SCA using echocardiography and compare results with controls. The study design was descriptive and cross sectional. Echocardiography was done to assess both systolic and diastolic function of subjects with SCA and controls. Eighty-six subjects with SCA and eighty-nine controls with haemoglobin A were recruited. The sickle cell group had a mean age of 27.6±7.6 while controls were 28.9±8.9. Right ventricular fractional area change (RVFAC) was not significantly different between both groups. However, tricuspid annular plane systolic excursion (TAPSE) and peak tricuspid annular systolic velocity (S) were increased in the sickle cell group as compared with controls. The sickle cell group also had more diastolic dysfunction (Transtricus...

Peripheral T Cell Lymphoma with Follicular Helper Phenotype, Case Report and Literature Review

T-cell lymphomas are a heterogeneous group of lymphoid neoplasms, with a broad classification; in which the peripheral T-cell lymphoma not otherwise specified, is the most frequently diagnosed subtype, demonstrating the complexity of this hematopoietic neoplasms, the lack of studies and knowledge, which difficult us to understand the behavior of this great spectrum of malignancies. We present the clinical case of a patient with late diagnosis and systemic involvement, with initial diagnosis of psoriasis and progressive evolution of the lesions to tumoral and exophytic nodular lesions predominantly in axillae and groin, associated with cytopenia and hepatic involvement, peripheral T-cell lymphoma with T follicular helper cell phenotype was confirmed by biopsy and immunophenotypic studies. This entity was recently described and included in the World Health Organization classification of neoplasms of the hematopoietic and lymphoid tissue updated in 2008. It is a rare entity difficult to ...

Cladribine an Alternative for Midostaurine in FLT3 Positive Acute Myeloid Leukemia

Midostaurin is registered for the indication newly diagnosed acute myeloid leukemia that is FLT3 mutation positive in combination with standard cytarabine and daunorubicine induction and cytarabine consolidation. The registration is based on a study of 717 patients that examined the addition of midostaurin 50 mg orally twice daily or placebo to standard induction on days 8 to 21 of a cycle for a maximum of two induction cycles. Survey of Pentazocine Addiction and Opioid Use in Adult Sickle Cell Anaemia Patients: The Perspective of Healthcare Providers

Background: Sickle cell anaemia (SCA) is characterized by episodes of bone pain creating a major role for analgesic therapy in the emergency management of this condition. Opioids, including pentazocine, is used in some cases and may be addictive with prolonged use. Aim: This study aims to assess the scope of opioid use; pentazocine addiction and availability of resources for its management. Materials and Methods: This study was conducted using a 10-item pre-tested and pre-validated questionnaire which was administered to medical doctors working in haematology units across Nigeria. Results: 55 doctors involved in SCA patient care in 25 Nigerian health institutions with 13,314 registered SCA patients responded. The 5-year prevalence rate of pentazocine addiction was 2.9%. In managing pain in these patients, pentazocine ranked 2nd (21.2%) as the drug of choice after NSAIDs (26.9%). Majority of the physicians (48.1%) frequently prescribed DF118 while about 80% of them rarely prescribed mo...
